JERSEY NUMBER FORM
An easy way to collect jersey number information before your team meeting. Especially important if you're taking over a new team or have lots of movement on the team.

GUEST PLAYER FORM
You'll need the player card of the player you're borrowing, plus a printed form showing the process was put through via the state or tournament. This is how to get that form. Takes 10 minutes.
